Office: Fees

All payments for services and supplements are expected the day of the service

All supplements will be in addition to the appointment fee.

The best time to make your appointment is the time of day when you are the most symptomatic. This is not always possible but give it a thought as you book your appointments. If you are unsure just call or email and I will let you know.

Paperwork

To get the most out of you time with the doctor it is best if you fill out your paper work ahead of time. The paper work can be found on the right hand column of this page. Otherwise, it is best if you arrive 15 minutes before your appointment to fill this out.

What you should you bring?

Bring any supplements you are or want to take. Bring any foods that you think may be causing you a problem. Just put a small amount in a zip lock bag. It is often helpful to bring small samples of the air where you spend time working and sleeping. Just open a small glass jar in that environment for 1 hour and then close it up. The water you drink is commonly helpful to test as well. It is helpful to test shampoos, creams, lotions, toothpaste….you are using. Any prior tests you have had performed should be brought in at the first visit.

Drugs/Prescriptions

If you are taking non-steroidal anti-inflammatories or anti-histamines it is best to not take them on the day of your appointment. If you are taking steroid such as prednisone, it is best to wait until a week after you are finished taking these drugs. If you have been on them for longs periods it may not be as much of an issue. These drugs may skew the muscle testing results or make the body seem less sensitive than it really is.

Out of Network:

Dr. Pietrantone does not participate (not in-network) with any insurance. We will provide you with a receipt that has your diagnosis codes, procedure codes and our tax ID upon request. If a patient is coming for multiple visits a summary receipt can be generated at the end of the month containing all visits. We are unable to fill out any paperwork for approvals, authorizations, etc. This would become too overwhelming if we had to do this for all patients. Submitting your receipts to your insurance does not guarantee coverage and reimbursement. This depends solely on your insurance plan’s coverage and limits.

How much will I get back?

This depends on your plan and if there is an out of network deductible that needs to be met. It is best to ask your insurance company how much they reimburse for chiropractic adjustments, if there are limits on the number of visits and if more documentation will be required. Some insurance companies may require forms and additional hoops to be jumped through. Due to time constraints I will not be able to fill these forms out.
